Overview

The increasing awareness on the varied consequences of hypogonadism in distinct organs and systems has supported the notion of estrogens as systemic agents. This observation is congruent with the variety of tissues affected by - trogens when used in hormone therapy formulations on hypogonadic women. Apart from the genital tract and the breast, recognized as traditional targets for estrogens, the skeleton, the vascular tree, or the central nervous system, are good examples of territories that have demonstrated sensitivity to estrogens. This evidence has created great interest, as shown by the great amount of lit- ature that has been produced on the benefits and risks associated with the use of estrogens. In parallel to the clinical interest, basic research has improved our kno- edge on the complexities involved in estrogen action at the molecular level. Together with effects mediated through specific receptors, a concept that has been the mainstay of the interpretation of estrogen action for years, there is enough evidence to hold the notion of receptor-independent effects. The substantial advances in modern technology applied to research have helped in enlightening the particulars of this versatile action of estrogens. This more detailed knowledge on the sophisticated mechanism of action of estrogens has nourished the emergence of multiple hypotheses speculating with the p- sibility of manipulating estrogen action. The notion that a widely extended regulatory system of cell function, as it is the estrogen receptor machinery, might be modulated at wish has arisen as an attractive, although still elusive postulate.

Description: This book describes the role of selective estrogen modulators (SERMs) in different clinical conditions and at the molecular level.
Purpose: The purpose is to provide an updated review of SERMs in the clinical and basic sciences, a worthy objective given new developments of the uses of SERMs in the field.
Audience: It is evident that the writers are authorities in their respective fields, targeting other professionals in the field of obstetrics and gynecology and endocrinology. Obstetrics and gynecology residents would probably need more summarized and to the point materials to take better advantage of this book.
Features: The book covers in depth the clinical and molecular aspect of SERMs in a very systematic fashion by including basic science in conjunction with clinical conditions.
Assessment: The strengths of this book are its subject matter and presentation in a systematic manner which makes it useful for professionals in this field. Also, the unique presentation of basic principles of SERMs action in the first part of the book makes it more useful. Figures are presented well, especially in the first part of the book, and are helpful for an understanding of the basis of molecular mechanisms. In contrast to the clinical part, the basic science part of the book is more summarized. At the same time, there are some weak points such as repetitive referencing in the same paragraph, especially in a clinical section (e.g. in chapter 12, one reference was cited three times for the same paragraph and subject), multiple instances of self referring (authors cited their own publications), unnecessary information, and such great detail that by the end of the topic, the clinical significance of the information is in danger of getting lost. In my view, the aim of a review book is not to transfer a summary of different papers, rather it is to interpret and summarize others' work in a systematic and objective way. If the authors summarize more succinctly the key points, this book would work well as a review.
